Consultation evaluation fees are required to be paid in full at the time of service, if you do not have insurance coverage. If you have insurance, we will file your insurance claim as a courtesy to you and no down payment is required for your consultation appointment.
For simple extractions, we will ask that you place a percentage down on the day of the procedure. The percentage amount required will be dependent upon your insurance policy’s annual deductible and yearly maximum allowable amount. We will file your insurance claim immediately following your procedure and any balance remaining that is not covered by your insurance company within 60 days will be your responsibility to pay in full. If your insurance company pays more than we estimate, we will refund you the credit amount.
For your convenience, we accept Visa, MasterCard, Discover and CareCredit. We deliver the finest care at the most reasonable cost to you and payment is due at the time service is rendered unless other arrangements have been made in advance. Most insurance companies will respond within four to six weeks after your claim is submitted. Please call our office if your statement does not reflect your insurance payment within that time frame. Any remaining balance after your insurance has paid is your responsibility. We will send you a statement. Your prompt remittance is appreciated.
