Oral Surgery Insurance Information
At Michaels Oral Surgery, we make every effort to provide you with the finest care and the most convenient financial options. To accomplish this, we work hand-in-hand with you to maximize your insurance reimbursement for covered procedures.
Oral surgery procedures are sometimes covered by both medical insurance and dental insurance. It is important that you bring your insurance information to your first visit.
Our office staff will help you understand the procedures covered by your insurance plan and assist in submitting your claim to your insurance as a courtesy to you. Professional services are rendered and charged to you, the patient, and not the insurance company.
Your insurance is a contract between you and your insurance company, not between our office and your insurance company.
What to Expect During Your Visit
We will ask for a down payment deposit, in the amount to be determined at your initial visit, to be applied to your account on the day of surgery. Any overpayments to your account will be promptly refunded. In the event that there is a balance due after your insurance pays, it is your responsibility to pay your account in full at that time.
If payment has not been received from your insurance company within 60 days, you will receive a statement at which time the balance will become your responsibility, and payment in full is expected. If you have any questions, please ask our staff. They are well informed and up-to-date. Our office can be reached at Michaels Oral Surgery Office Phone Number 740-654-6628.

What to Bring to Your Appointment
Please bring your insurance information with you to the consultation so that we can expedite reimbursement. You are responsible for checking with your employer to determine if your surgery will fall under your medical or dental coverage.
You should know the name and address of your medical and dental insurance companies and whether or not they require a predetermination and/or referral from your primary care physician. If you have more than one insurance, you need to inform us who your primary insurance carrier is.
We are in-network with:
- Anthem Grid +
- Cigna PPO
- Connection Dental
- Delta Dental Premier
- Metlife PDP Plus
- Multi Plan
- Superior Dental
We do not participate with Medicare & we do not accept Medicaid.
Medicare will not provide benefits for dental surgery. If a Medicare patient chooses to be seen at our office, payment is due in full at the time of service.
Since we do not participate with Medicare, patients with Medicare insurance cannot bill their date of service to Medicare or to a Medicare secondary insurance.
